Sleep disturbances and related disordered breathing after hip replacement surgery: A randomised controlled trial

چکیده
Mechanisms of postoperative sleep architecture disturbances and sleep-disordered breathing are uncertain. The authors hypothesised that patients undergoing surgery under regional anaesthesia without opioids used for analgesia would experience lesser changes in these parameters than operated general with per- opioids. After ethical approval informed consent, total hip replacement were included a prospective, randomised trial comparing 3 groups patients: (1) S-LPB group receiving spinal by lumbar plexus block; (2) GA-PCA morphine patient-controlled analgesia; (3) GA-LPB block. Outcome measurements polysomnographic breathing. Eighteen completed the 5-night study protocol (preoperative night: N-1, nights: N1 to N4). percentage rapid eye movement (REM) decreased 49% 47% during respectively. A rebound phenomenon more 40% 25% was observed N2 N3. Apnoea hypopnoea index (AHI) number arousals per hour significantly increased N3 when compared N-1 GA-groups. No no measured group. Postoperative pattern disturbed GA groups. Both preserved associated free opioid analgesia.

Sleep-Disordered Breathing After Stroke
Sleep-disordered breathing is common after stroke and most often presents as obstructive sleep apnea (OSA), with an estimated prevalence between 43% and 70% compared with 4% and 24% in the general population. OSA is the result of partial or complete closure of the upper airway while attempting to breathe during sleep. It is characterized by snoring, hypopnea, and apnea with intermittent awakeni...
متن کاملBreathing frequency in sleep related respiratory disturbances.
In normal adults, breathing frequency (f) is highly reproducible within an individual ranging from 8 to 25 min-1; during sleep, f is known to change only minimally. This variable is rarely reported in studies of adults with various sleep related respiratory disturbances (SRRD). We occasionally observed a spectacular increase of f during sleep in patients with SRRD associated with hypopnea and 0...
متن کاملSLEEP DISORDERED BREATHING Randomised controlled crossover trial of humidified continuous positive airway pressure in mild obstructive sleep apnoea
Background: It is unclear whether continuous positive airway pressure (CPAP), the treatment of choice for severe obstructive sleep apnoea (OSA), is effective at improving outcomes in mild OSA. Methods: To help define the role of humidified CPAP in mild OSA, a randomised crossover study was undertaken of patients with an apnoea hypopnoea index (AHI) of 5–30/hour. Subjective sleepiness, objective...
متن کاملSkin closure after total hip replacement: a randomised controlled trial of skin adhesive versus surgical staples.
We undertook a randomised controlled trial to compare the outcomes of skin adhesive and staples for skin closure in total hip replacement. The primary outcome was the cosmetic appearance of the scar at three months using a surgeon-rated visual analogue scale. In all, 90 patients were randomised to skin closure using either skin adhesive (n = 45) or staples (n = 45). Data on demographics, surgic...
متن کاملEvaluation of an alternative care provider clinic for severe sleep-disordered breathing: a study protocol for a randomised controlled trial
INTRODUCTION Despite the high prevalence of sleep-disordered breathing (SDB) and the significant health consequences associated with untreated disease, access to diagnosis and treatment remains a challenge. Even patients with severe SDB (severe obstructive sleep apnoea or hypoventilation), who are at particularly high risk of adverse health effects, are subject to long delays. Previous research...
