Sleep disturbances and related disordered breathing after hip replacement surgery: A randomised controlled trial

Mechanisms of postoperative sleep architecture disturbances and sleep-disordered breathing are uncertain. The authors hypothesised that patients undergoing surgery under regional anaesthesia without opioids used for analgesia would experience lesser changes in these parameters than operated general with per- opioids. After ethical approval informed consent, total hip replacement were included a prospective, randomised trial comparing 3 groups patients: (1) S-LPB group receiving spinal by lumbar plexus block; (2) GA-PCA morphine patient-controlled analgesia; (3) GA-LPB block. Outcome measurements polysomnographic breathing. Eighteen completed the 5-night study protocol (preoperative night: N-1, nights: N1 to N4). percentage rapid eye movement (REM) decreased 49% 47% during respectively. A rebound phenomenon more 40% 25% was observed N2 N3. Apnoea hypopnoea index (AHI) number arousals per hour significantly increased N3 when compared N-1 GA-groups. No no measured group. Postoperative pattern disturbed GA groups. Both preserved associated free opioid analgesia.
